Payroll Deduction Form

Please note the following:

• Forms turned in by June 6th will be based on a 12-month schedule.
• The schedule decreases by one month for every month that passes. (i.e. If you submit your form in September, your contribution will be based off of 9 months vs. 12 months).
• Only employees that are paid monthly are eligible to participate in payroll deductions.
• Payroll requests are processed on the 15th of each month - requests received after the 15th will be processed the following month. 

Employee Discounted Levels Giving Levels below reflect the discounted rate:

Chiefs Club:

• Indian - $75 
  
• War - $150
  
• Tribal - $300
  
• Super Chief - $500
  
• Loyal Chief  - $925.50
  
• Golden Chief - $1,250
  *100% of Chiefs Club gifts are tax deductible.

Blue Masque Group:

• Producer’s Circle - $900 and up
  
• Playwright’s Circle - $400-$799
  
• Director’s Circle - $200-$399
  
• Myrtle’s Friends - Any Amount/No Tickets
  
• YADF Donor - Any Amount/No Tickets
