Instructions
This form is for high school students CURRENTLY in 9th, 10th, 11th, or 12th grade.
The DISCOVER application contains four sections:
- Student Information
- Mentor Information
- Parent Information
- Medical and Consent Forms
NOTE: There is a $150 Registration fee required to hold your child's spot at Discover. If this fee presents a hardship for your family, please register below and follow the instructions on the following screen.
