Apply for a Clergy Thriving Grant

Please provide complete and accurate information below, with as much detail as possible. You must be a current member of the Catawba Clergy Network (CCN), to apply. 

Before you begin, download and complete the W9 form - you will need to upload it below. Please be sure the W9 you upload is for the person/organization to which you’d like the grant check sent - Thriving Grants are typically issued to the Congregation. We cannot award a grant without the W9 form.
